Aging In Place
This program is designed for seniors who have been diagnosed with dementia and want to stay in their home as they age. We can help the family member or caregiver of a senior with dementia by providing the education they need.
This program will help set up the home for successful and safe aging, allowing your loved one to grow old while living life to the fullest in the place where they are the most comfortable.
How Does It Work?
The program starts with an in-home assessment. An Occupational Therapist (OT) visits the home. They meet with everyone involved, including the caregiver(s). They assess the home, and will suggest appropriate services and any modifications that are necessary.
After the assessment, the OT creates a plan, recommends home modifications, and creates an education package for all the people that are involved in the care process.

Fall Risk Assessment
A fall can be a serious event. At its most severe it can be life-threatening. Regardless of its severity, a fall is definitely life-altering. That’s what makes a fall risk assessment so vital.
A fall risk assessment is an evaluation of both your physical health and the safety of your home.
Physical Fall Risk Assessment
A physiotherapist from our team meets with you. They ask questions about your overall health, including previous falls and problems with balance, standing, and walking. They also test your strength, balance, and gait through a series of exercises.
After the physical assessment, the physiotherapist recommends an exercise program and strategies to prevent falls and reduce the chance of injury.
Home Risk Assessment
Your home environment also plays a major role in the risk of a fall. That is why we assess your home, inside and out.
An occupational therapist reviews the interior and exterior of your home, paying special attention to tripping hazards, elevations, lighting, and transfer spots. A report is provided with any recommendation for necessary home modifications.
These two assessments will set you up for successful aging in your home by helping to reduce the possibilities of a fall.

Abilities In Action For Seniors
Are you a senior and have an upcoming surgery? With our Abilities In Action Program, you can get back to the activities you love more quickly.
It starts with a pre-surgery consultation with a physiotherapist from our team. They specialize in osteoarthritis and will guide you in a pre- and post-surgery exercise and strengthening program. Do the exercises at home or take advantage of our senior-friendly gym (for a nominal fee).
